Financial Aid Forms

Students must apply annually for financial assistance.

1. All financial aid applicants must complete the UDC-DCSL Financial Aid Application Form (.pdf) annually.



2. Both first-time and continuing loan borrowers must complete the Free Application for Federal Student Assistance (FAFSA). UDC-DCSL's FAFSA Code is B08083.



3. Students who choose to borrow a Federal Stafford loan through a lender other than The Access Group must complete and submit to the Office of Financial Aid a hard copy Lender Request Form (.pdf) and Master Promissory Note (.pdf).



4. Supplemental Information Form (.pdf): Optional form for students with special circumstances, i.e., child care or medical issues, significant change in income from reported in the FAFSA base year.



5. Continuing Student Merit Scholarship Application (.pdf): Merit scholarship application for upper level students. Must be submitted with all supporting documentation on or before March 31.
